The short answer is yes, progesterone can be tested. However, the longer answer is that progesterone testing is rarely as straightforward as many women expect.
Unlike cholesterol or iron levels, progesterone changes significantly throughout the menstrual cycle. A result that appears low on one day may be completely normal on another. In addition, progesterone levels naturally fluctuate from month to month, particularly during perimenopause when ovulation becomes less predictable.

The most helpful question is often not:
“Which test is best?”
but rather:
“Which test is most appropriate for me at this point in time?”
In my clinical experience, no hormone test can replace a detailed clinical history.
Tests provide useful information, but they represent only one piece of the puzzle. Symptoms, menstrual patterns, sleep quality, stress levels, metabolic health, nutrient status, and lifestyle factors provide equally important clues when assessing hormone health.
The Day 21 Myth
If you’ve ever searched online for progesterone testing, you’ve probably come across the advice:
“Test your progesterone on Day 21 of your cycle.”
While this recommendation is well intentioned, it is not always correct.
Day 21 testing is based on one important assumption: that you have a regular 28-day menstrual cycle and that ovulation occurs around Day 14.
In reality, many women do not have textbook 28-day cycles. Some ovulate earlier, while others ovulate much later. This is particularly common during perimenopause, when ovulation becomes less predictable.
Current WHO guidance (2025) notes that mid-luteal progesterone testing should generally be performed approximately seven days before the expected onset of menstruation, recognising that the specific cycle day varies according to cycle length.
